Exposure to COVID-19

Self-Reporting & Next Steps:

It is important that we hear from you!

To help ensure the health and safety of the campus community, the University requires students and employees to complete the self-report form if they are experiencing one or more of the following:

  • Diagnosed/being treated by a healthcare professional for COVID-19
  • Exposed to a known positive case
  • Experiencing symptoms of COVID-19

Based on whether you (1) have been diagnosed with COVID-19; (2) have been exposed to a known positive case; or (3) are experiencing symptoms of COVID-19, please review the following guidance.

  • 1. Positive COVID-19 diagnosis

    A.  No Symptoms

If you test positive for COVID-19, but have no symptoms, you must self-isolate for five (5) days after the date of your positive diagnosis and then wear a mask when around others for an additional five (5) days.  During your 5-day isolation, stay home and do not go to work or school. Do not participate in any on campus activities or attend any events. If you must leave your home for essential needs, like medical care or food ensure that you wear a mask or face covering while out in public and adhere to strict physical distancing of at least 6 feet away from others.

    Employee:

Please contact your supervisor to notify them of your required self-isolation. You may automatically return to work after completing the isolation procedures described herein so long as the conditions for leaving isolation have been met.  If you have questions or concerns about your clearance status, please email covid19@utulsa.edu.

If you remain asymptomatic, you may leave isolation after 5 days. Day 0 is the day of your positive test. If you develop symptoms after testing positive, your 5-day isolation period must start over.   Day 0 is your first day of symptoms. Follow the guidance in Section 1(B) below.

Complete this form to identify your close contacts.

    Student:

Please reach out to your professors to make arrangements to complete your classwork during your self-isolation. If you need assistance, please contact Student Access at studentaccess@utulsa.edu.

If you live in a Residence Hall or Greek Housing, you must temporarily vacate your housing during your isolation period. Apartment-dwellers do not need to relocate. Whenever possible, we urge students to isolate themselves at their permanent home, but we recognize this will not be an option for everyone. If available, the University maintains a limited number of isolation spaces as an option should a student living in university-owned housing need to temporarily relocate during an isolation period. For more information on isolation housing, please contact Campus Security at 918-631-5555.

If you remain asymptomatic, you may leave isolation after 5 days. Day 0 is the day of your positive test. If you develop symptoms after testing positive, your 5-day isolation period must start over.   Day 0 is your first day of symptoms. Follow the guidance in Section 1(B) below.

    B.  Symptoms

If you experience or develop symptoms of COVID-19 after testing positive for COVID-19, you must self-isolate until five (5) days after the onset of symptoms (Day 0 is your first day of symptoms). You may then leave isolation if you are also 24 hours with no fever without the use of fever-reducing medications and so long as your other symptoms are improving. After your 5-day isolation period, you must wear a mash when around others for an additional five (5) days.

During your 5-day isolation, stay home and do not go to work or school. Do not participate in any on-campus activities or attend any events.
