Official Notice: Immunization Requirements for University of Tulsa Students
Oklahoma state law requires that all students who attend Oklahoma colleges and universities provide proof of immunization for certain diseases. Medical, religious and moral exemptions are allowed by law.
Acceptable documentation of immunizations includes any of the following:
- Signature of a physician on the Immunization Record Form
- Copies of shot records
- Copies of medical records
- Copies of school health records
- All information must be submitted in English, with dates of actual immunizations
Immunizations Required by Law:

* Oklahoma law requires that all students who reside in on-campus housing be vaccinated against meningococcal disease UNLESS 1) the individual signs the TU Meningococcal Vaccine Waiver, or 2) in the case of a minor, the individual’s parent or guardian signs the waiver.
FAILURE TO COMPLY WITH THESE REQUIREMENTS WILL RESULT IN A HOLD BEING PLACED ON FUTURE ENROLLMENT.
The Meningitis and Hepatitis B vaccinations are available at the AHC and local pharmacies. available at many local pharmacies, Community Health Connection or your primary care provider.
