Please upload a copy of your COVID-19 vaccination record card to the Alexander Health Center (AHC) Patient Portal. COVID-19 vaccination records are securely stored in The University of Tulsa’s third-party Medicat platform. Medicat is a HIPAA-compliant health records database accessible only by licensed professionals in the AHC. The AHC protects patient privacy and your personally identifiable information remains private at all times.
Each week, the AHC produces a report on the number of TU employees who have submitted their vaccination records. TU publishes these aggregate data on our COVID-19 Dashboard. Individuals’ names are not reported.
Uploading your vaccination record card is voluntary; however, we strongly encourage all employees to do so in order for us to have as clear a picture as possible of the extent of vaccine protection in our university community. TU’s COVID-19 management team uses these valuable data to make better decisions about ongoing campus operations.
Here are the steps to take in order to upload a copy of your COVID-19 vaccination record card:
- Take a photo of your vaccination record card using your phone.
- Go to the AHC patient portal.
- Click on “Upload” (located near the top of the page on the right-hand side).
- In the blank rectangular box, click on the down arrow to open a dropdown menu.
- Choose 2021 COVID VACCINE DOCUMENTATION.
- Click on “Select File.”
- Choose the photo of your COVID-19 vaccination record card on your device.
As of March 9, 13.3% of TU employees reported to the AHC that they have received at least one vaccine dose, with 6.9% of those employees reporting that they are fully vaccinated.
